This does not suprise me in the least. As a nurse for over 26 years the quality of care in this state is pathetic. I refuse to nurse in this state and go back to CA to work. There needs to be a state quality board that has metrics for improvement and some leadership. It's the wild wild west out here were physicians are running roughshod.
As far as the retained foreign bodies my guess there is P&P for OR's . If there are procedures done in other parts of the hospital, this policy is probably not even covered, just like the "wrong side" p&p were other parts of the hospital other than the OR are doing procedures are not adhering to these policies.
Also many adverse events are not reported because of punitive actions by managers who are not there for lessons learned. Instead of doing a root cause analysis to see where the "system" has failed, they fire the people involved...Most managers do not think about "system failures" and they get the non-reporting because of their actions.
