I discussed this matter (whether A1C could be used to diagnose diabetes) with a pathologist one time. He said A1C is useful to monitor diabetics, but it is not used to diagnose diabetes. I was surprised to read that Dr. Olson thought A1C would be useful to find new diabetics (but I want to learn more).
I live overseas and I am interested in health education and health screening (especially in rural settings). Blood glucose testing is easy, but not cheap. Urinalysis was thrown out years ago as a way of monitoring diabetics because it is not reliable for insulin dosage decisions. I am wondering, however, if urine glucose would be a useful screening tool in rural free clinics (or possibly in urban setting as well to find new diabetics). For mass screening, I think the "rule of thumb" is to do a blood glucose on everyone who is "over forty" or overweight (if the organizers have the funds to buy the strips). I am interested in readers' comments (it is good to view the same issue from different angles).
