This post is important because it reports that less mutilating surgery can be as curative as a modified radical mastectomy in EARLY stage breast cancer. This is a step forward in the surgical treatment of breast cancer. It has nothing to do with POT. Women will be able to undergo surgery that does not require that they lose a breast to have a chance at a cure for cancer, as long as the primary tumor site is small and the axillary nodes are extensive.

Typical of the 'slash and burn' approach to cancer. A method that has not changed in 30 years. Europeans were also first to use lumpectomy's followed by local radiation instead of mastectomy's with very similar results in survival. the medical field for all its accolades is still a cut and run buisness. I truly feel sorry for most women that have to be subjected to the current medical process.

The specialists who treat patients will be watching them very carefully. Women must have heavy radiation, and/or chemotherapy, or take a very expensive Prescription Substitute for Chemo for many years.. After a Lumpectomy, radiation, chemotherapy are done, also.

Oh, in case you did not get my previous post, I am in favor of the limited lymph node remove approach. The less cutting the better. The only problem is the fear of the patients and the fear of the doctor of returning cancer and failure. Something no one could know, but act on every day. Stop the mutilation of women!!!! You are correct. The old original radical mastectomy performed by Halstead involved the removal of the breast, all axillary nodes and the pectoralis major muscle. This model was adhered to by surgeons for years. Only a few women are still alive that had this mutilating surgery. If you ever see a picture of one, the ribs can be seen through the skin and the skin is paper thin due to compromise of its blood supply. This surgery was still being done in 1976, or later. Too many women still have to go through too much surgery. Advances are being made, just slowly. A lot of times too slowly. My father lost his leg to a sarcoma that now days would not have required that much surgery. He was cured but later had a heart attack that ultimately crippled him even more. I hope for a day when a single shot or pill will cure every kind of cancer. I just hope we can afford it when it comes.

I sometimes wonder if she would still be with us if she had started with the more invasive surgery, but of course we will never know. A woman needs as much good information as possible and support for whatever decision she makes, but I'm not sure how much this helps.

As an 8 year fortunate one with stage II, I am living with the reminders everyday. All I could think of is get it all out and do not let me think it is still here or that it will come back. The first time was a lumpectomy and radiation. After 11 months my doctors tells me they did not get it all. With out thinking this time I said get them both out of here. I am so glad I did. Too many people IKnow are not around. Either they went the less aggressive forms of chemo or it was too late and spreading. Yes my one arm pit is numb and irritated by a bra. Yes, I wear a bra. I had reconstructive surgery that created new ones from my lower abdominal. Painful procedure (1 year of hell), but gave me my dignity.
Take all the treatment you can. Pick an aggressive surgeon, plastics and oncologist doctors. Any doubt run and find somebody else. Drill the hell out of them. Do your home work. They are not God. Exceriscise and eat a low fat diet. Pray like never before and thank God there are cures out there.
One has to appreciate the stories of failed lumpectomy approaches. However, the studies done in Europe showed no statistical difference. I know, it IS cold to reduce survival to statistics, seems very insurance oriented thinking. But there are also many women that have been through radical removals and died anyway. They, however, are not here to speak their side. In the end all we have is the statistics to guide our decisions. An informed patient is an empowered one. That is still not any guaranty of success. I had a mastectomy AND all my nodes on that side under the arm removed. My tumor was invasive, but none of the nodes were positive for cancer, and yes, I developed lymphodema on that side in the arm, but, hey, after nineteen years, I am still alive and well, no recurrance of cancer of any sort. After the initial surgery, I had six months of chemotherapy. I was under the care of Vincent Caggiano, and Gregory Graves was my surgeon. I owe those men my life. So, thank you Vince and Greg. I love you both.
Oh, and remember, when reading about statistics, yes, maybe most of the women who have the limited surgery and selected removal of lymph nodes survive, but what if you are in the 15 % or whatever the percentage is, that die? Statistical templates can be difficult to understand, and of course, don't mean jack to the women who die.
The 15 per cent could be the women who have the Virulent type of Breast Cancer. Nothing can be done to cure them. It is a very fast moving breast cancer, or it eventually goes to the bones, also.. They have no choice.
The women who find out they have this type of BC, know, like Ovarian Cancer, will pass away very quickly, choosing not to spoil what's left of their quality of life with chemo/or radiation.
15% die with the more aggressive surgery as well. There is no 100% cure rate for breast cancer, or anything else for that matter. Patients who undergo a less extensive form of surgery and have a recurrence still have a significant chance at a cure with more aggressive surgery if it becomes necessary. This article was written about a subgroup who had less extensive cancer, but there are patients who have very aggressive cancers that will recur no matter what the surgical approach. High grade tumors (grade 3/3), estrogen receptor negative tumors, progesterone receptor negative tumors and now days HER2neu negative tumors are the ones that have the highest risk of recurrence and the highest mortality rates. So are inflammatory breast cancers.
To show how chemotherapy has advanced, it is now given for approximately 36 weeks and often given one day out of every 3 weeks. When it was first used, it was given daily for 2 years. Also, before Herceptin was developed, patients who had HER2neu antigen on their tumor cells were likely to die because this antigen is associated with resistance to chemotherapy. Far more of them died from metastatic disease. Now, these patients get 3 to 6 months of chemotherapy plus Herceptin and then a year of Herceptin alone and their chance of cure is better than that of women who have HER2neu negative cancer. There are so few of them relapsing that it is hard to do studies on how to treat the ones that do relapse.
It is a woman's prerogative to choose the type of surgery she undergoes. What researchers are trying to do is come up with alternative approaches so the women (and the few men) that get breast cancer have choices they can make. If one wants bilateral mastectomies that can be done, but if one chooses less surgery it can be done also. In either case, the key is to be followed-up closely to try to catch a recurrence early. Unfortunately there are still those in the 15% catagory that have cancers that recur with all the treatment currently available but that is still a lower number that 35 years ago and that % will continue to fall with further research and newer treatments.
I encourage people to examine all the available treatments for cancer. I believe that people should know all about surgery, radiation and chemotherapy. This is even less surgery than lumpectomy "NUKE". It does not require an extensive node dissection that can lead to severe edema of the arm.

To show how chemotherapy has advanced, it is now given for approximately 36 weeks and often given one day out of every 3 weeks. When it was first used, it was given daily for 2 years. Also, before Herceptin was developed, patients who had HER2neu antigen on their tumor cells were likely to die because this antigen is associated with resistance to chemotherapy. Far more of them died from metastatic disease. Now, these patients get 3 to 6 months of chemotherapy plus Herceptin and then a year of Herceptin alone and their chance of cure is better than that of women who have HER2neu negative cancer. There are so few of them relapsing that it is hard to do studies on how to treat the ones that do relapse.
It is a woman's prerogative to choose the type of surgery she undergoes. What researchers are trying to do is come up with alternative approaches so the women (and the few men) that get breast cancer have choices they can make. If one wants bilateral mastectomies that can be done, but if one chooses less surgery it can be done also. In either case, the key is to be followed-up closely to try to catch a recurrence early. Unfortunately there are still those in the 15% catagory that have cancers that recur with all the treatment currently available but that is still a lower number that 35 years ago and that % will continue to fall with further research and newer treatments.
Chemotherapy, radiation and surgery are horrifically painful and disfiguring "treatments" for cancer that kill more often than they cure.
