And before some do-gooder tells me it's bad to smoke, have I got some filthy confined spaces for you to work in, if you can handle the physical strain. Health risks from smoking are among the least of my worries. I hope I make it to 60.

Duh, if they have to yell to have a conversation, then they aren't wearing their hearing protection. Maybe plugging their ears is causing constriction of blood vessels that cuts off circulation to the brain, which in turn cripples the heart. This seems like silly science.
What makes you think that putting muffs on your ears protects the heart? It isn't necessarily the sound pressure on the ear drum that causes the association with heart risk. My uneducated guess is that it would it either be from emotional stress or micropressures affecting the heart organ itself.
What makes us think that noise injures the heart? I find the whole thing to be somewhat questionable. Maybe it's sound vibrations on the whole body. If that's the case, we better throw away our cell phones and telephones.
I was a professional welder for 10 years until I learned that the trade no longer commands the wage and respect it deserves. I found that many other jobs pay just as much or more and you don't have to breath smoke, get burned, etc...
And you don't even have to pay me to tell you that it may not increase the risk.
If it really does.....wear protective gear.
If you need to wear protective gear, perhaps you shouldn't be doing it.
Welders are at a high risk for more than just heart problems; they also breathe toxic fumes.
And there are full protective suits for welders that are even supplemented with cold air for hot conditions.
But that's beside the point. The protective gear I am referring to is ear plugs......Pretty cheap.
Wear protective gear, easier said than done especially in a combat zone! The one industry that they did not mention was Aviation I wonder why?
