In the area of Northern Michigan where I grew up everyone knew the local doctor and usually his family as well. That has all changed now as the young locals who become doctors never come back to their hometown electing to practice medicine in the cities where they can make more. Now you can't understand the MD's that have moved into town, most are from India or Pakistan and have no connection to that part of the world at all. They come on J1 visa's as a shortage exists in medicine in these areas. I'm sure most are excellent doctors, it just feels like we as a nation have lost something valuable an we will never get it back.

No surprise. Rural areas have seemingly, almost always been behind the urban areas, in most social issues.
